SMART Mercury Removal
Safe Mercury Amalgam Removal
As the longest standing IAOMT member and only Biological Dental Accredited dentist in Nebraska rest assured your care is in experienced hands.
For patients who choose to have existing mercury amalgam fillings removed, Dr. Allen follows the SMART (Safe Mercury Amalgam Removal Technique) recommendations developed by the International Academy of Oral Medicine and Toxicology (IAOMT).
As an IAOMT accredited member and SMART-certified dentist, Dr. Allen incorporates additional precautions during amalgam removal that are designed to minimize potential exposure to mercury vapor and amalgam particles.
Our goal is to provide a carefully controlled approach to amalgam removal while protecting both the patient and dental team throughout the procedure.
Why Consider Mercury Amalgam Removal?
The American Dental Association (ADA) say that the level of mercury vapor exposure from silver fillings are too low to cause concern. But mercury itself is classified as a neurotoxin. Over time mercury fillings do in fact release small amounts of toxic vapor into your body every time you eat, drink, brush your teeth, and clench your jaw. With so much of the population suffering from neurological, immunological, or simply unknown illnesses is it really worth the risk?!
In the past mercury fillings were the only option available. Since it is a metal-based material they expand and contract due to changes in temperature (just like potholes on a road) and may eventually cause cracks in teeth. At our practice, we believe patients should have the opportunity to understand the available materials and make informed decisions about their dental care. *Our dental fillings use BPA-free materials.
Patients may choose to discuss amalgam removal because of:
-
A desire for mercury-free or metal-free dental restorations
-
Concerns about mercury-containing materials
-
A damaged, fractured, or failing amalgam restoration
-
Recurrent decay around an existing filling
-
Cosmetic concerns
-
A preference for tooth-colored or ceramic restorative materials
An existing amalgam filling does not automatically need to be removed simply because it contains mercury. We evaluate each restoration individually and discuss whether replacement is appropriate for your particular circumstances.
What Is SMART Mercury Removal?
SMART stands for Safe Mercury Amalgam Removal Technique.
The SMART protocol was developed by the IAOMT and provides recommendations for dentists who choose to incorporate additional precautions when removing dental amalgam.
These precautions are intended to minimize potential exposure to mercury vapor and amalgam particles during the removal process.
As research, equipment, and clinical techniques continue to evolve, recommendations may also change. Our practice follows applicable IAOMT/SMART recommendations and incorporates appropriate precautions into the removal process.
Our SMART Mercury Removal Protocol
During SMART amalgam removal, Dr. Allen uses multiple precautions designed to reduce exposure during treatment.
Amalgam Removed in Larger Pieces
This is the part of the procedure where a skilled and thorough dentist is most important. The amalgam is sectioned and removed in larger pieces rather than unnecessarily grinding the restoration into smaller particles.
This helps minimize the amount of amalgam that is disturbed and airborne during removal.
Keeping the Filling Cool
Cool water is used during the procedure to help control heat and reduce the amount of vapor generated during removal.
High-Volume Suction
High-volume evacuation is positioned near the treatment area to capture water, debris, and airborne material generated during the procedure.
Additional Air Filtration
Specialized evacuation and air-filtration equipment is used to help reduce airborne contaminants in the treatment environment.
Our practice uses an "elephant trunk" style external suction system as an additional measure during SMART amalgam removal.

Dental Dam
A dental dam may be used to isolate the tooth being treated and help prevent amalgam particles or debris from entering the rest of the mouth.
Alternative Air Supply
An alternative air source will be provided during treatment so the patient can breathe through the nose rather than directly through the treatment area.
Protective Coverings
Additional protective coverings may be used to help protect the patient's face, clothing, and body from water, debris, and restorative material during the procedure.
Replacing the Amalgam Restoration
Once the amalgam has been removed, the tooth is carefully cleaned and prepared for its replacement restoration. *Our dental fillings use BPA-free materials.
Depending on the condition and location of the tooth, replacement options may include:
-
Tooth-colored composite
-
Porcelain
-
Ceramic restorations
-
Other appropriate restorative materials
Before treatment, we discuss the available options so you understand the material being recommended and why it may be appropriate for your tooth.
A Thoughtful Approach to Mercury Amalgam Removal
We recognize that patients have different concerns and preferences regarding dental materials. Our approach is to provide clear information, evaluate each restoration individually, and respect your choices.
If amalgam removal is appropriate and you decide to proceed, SMART Mercury Removal provides a structured protocol with additional precautions designed to minimize potential exposure during the procedure.
